About the Activity

The evolution of treatment options for HIV-infected patients has both simplified and complicated the selection of an initial ARV regimen. PI-based regimens have always been associated with the absence of PI resistance mutations and with less NRTI resistance at the time of treatment failure. The rationale for using initial NNRTI-based regimens, on the other hand, has been their efficacy and simplicity. A number of recent head-to-head trials have illustrated the complexities involved in comparing the outcomes associated with these initial regimens and the effect of these regimens on future treatment options.

Learning Objectives

After completion of this educational activity, the participant should be able to

  • explain the importance of resistance testing both before treatment initiation and after virologic failure

  • differentiate initial ARV regimens in terms of the evolution of resistance among patients with virologic failure

  • evaluate recent research findings related to the selection of viable first-line regimens for HIV-infected patients

  • outline the advantages and disadvantages of NNRTI-based and PI-based initial therapy

  • remove patients experiencing early virologic failure from nonsuppressive ARV regimens
